Advance Care Planning

Serious Illness, Injury, and Disability can happen at any age.  Learn how to plan ahead to protect your rights and wishes.  

During this interesting and informative workshop you will learn about the umbrella of Advance Care documents: Representation Agreements; Advance Directives; understanding hospital and residential care medical orders of intervention; No CPR orders; and Medical Assistance in Dying.

Facilitator Connie Jorsvik was an RN for 25 years, is a professional healthcare navigator and holds her certificate in Advance Care Planning.  She is a local, provincial and national speaker on ACP.
